Development of an electronic register for managed care of
chronic diseases in north-west London

abstract
General practitioners in three primary care trusts are using a
shared chronic-disease register to help manage coronary heart
disease and diabetes in the north west of London. The register is
compiled from data on over 200 000 patients that is contributed
by 97% of the trusts’ general practices.
At least 12 of 14 measures of data quality and process and
outcome standards have improved. The authors believe that use of the
information from the chronic-disease register has made a significant
contribution to improvements in managing CHD and diabetes care that
could not have been achieved without the help of IT.
Further IT and associated service developments are also
discussed.
